6. What happens once I refer an adolescent to the I AM Program?-

Once a referral is made, the I AM Program Coordinator will help identify a funding source. Once a funding source has been identified the following information must be submitted to TEEG in order to complete an assessment:

  1. A brief description of the adolescent
  2. A brief description about the family
  3. A brief description of past and current functioning status
  4. A signed approval for the assessment fee.
  5. A release allowing the program to speak to the family

Once all of the abovementioned information is received a mentor is identified. TEEG schedules and conducts an assessment with the adolescent and family.

After the assessment an Assessment Report (a report describing the adolescent’s situation, needs, allergies, diagnosis, meds, interests etc) is written. A Treatment Plan is developed (A written plan identifying the adolescent’s needs and task specific goals to address these needs.) A Financial Contract itemizing all fees associated with providing the service is developed. Then the Treatment Plan and Financial Contract are all submitted for approval to the Funding Agency. Once documentation is approved by all parties the services begin.

7. How long does it take to begin services?-

Once the assessment fee is approved by the funding agency TEEG conducts an assessment as soon as all necessary parties are available to partake in the assessment.

Once the assessment is complete the necessary documentation is written and submitted to the funding agency for approval. Once TEEG receives the approval from the funding agency, services begin.

8. How much time will an adolescent spend with the identified mentor?-

Each case is designed to meet the needs of the individual. The amount of hours the mentor will spend with the adolescent each week is determined at the time of the assessment. The typical unit entails the mentor to spend 5 hours each week with or on behalf of the adolescent. The amount of time spent with the adolescent is case-specific and may increase or decrease depending on the needs of the adolescent.

9. What types of activities will an adolescent participate in with a mentor?-

Activities that mentors and participating adolescents engage in together include but are not limited to:

  • Mini-golf
  • Attend sporting events
  • Mall visits
  • Participate in a Therapeutic Sailing Program
  • Hiking
  • Laser Tag
  • Bowling
  • Sporting events
  • Community events

10. How long would an adolescent have a mentor?-

The length of time the adolescent remains involved in the I AM Program is dependent on the adolescent’s needs. Most units of service are 16 weeks long with a case review around the 10th week. At that time, the mentor, clinical supervisor and I AM Program Coordinator decide whether to request another unit of service based on the progress thus far reached during the first unit of service. This continues for as long as the adolescent needs the service. The average length of time an adolescent participates in the I AM Program is about 1 year.  On rare occasions, a unit may be shorter than 16 weeks.

-

11. What are the qualifications of the mentors?-

TEEG mentors are motivated individuals with knowledge of adolescent mental health and family dynamics. TEEG mentors meet the educational requirements of having a Bachelor’s Degree in Social Work or closely related field or have significant life experience.
