This is a community-based program, serving adolescents in need in the 10-town
area. The program aims provide the following:
- Connect adolescents to resources in their community
- Establish relationships between adolescents and natural community supports
- Promote family stabilization
- Promote and maintain successful in-home placements
- Promote educational success, self-confidence and individuation
Adolescents in the program are provided the opportunity to participate in a multitude
of activities to expand their skill base, increase positive peer interactions and develop
undiscovered talents. Each adolescent has an individualized treatment plan designed to
address their specific needs, and build on their individual strengths.
Adolescents are assigned a mentor who works closely with the adolescent in the community.
Mentors collaborate with community providers to maintain consistent treatment goals in
all aspects of the adolescent’s care. Mentors also focus on families. Mentors provide
crisis intervention and support for the guardian/parent. Mentors help to navigate the
healthcare systems in which the adolescent is involved, while providing education on
balancing nurturance and structure within the home environment.
A Child Psychologist and/or a LCSW provide clinical supervision to each mentor on a
weekly basis.
